When is a Chest X-Ray ordered?

What's in a 5C Chest X-Ray report
A standard 5C Chest X-Ray report is structured to be readable by the referring clinician, the hospital admin reviewing TAT, and any auditor checking NABH compliance.
Clinical history
Brief context: presenting symptoms, prior conditions, referring clinician's question, and whether the study is for screening, follow-up, or acute workup.
Technique
Views obtained (PA, AP, lateral), patient positioning (erect or supine), and any limitations such as rotation, under-inspiration, or motion.
Comparison
Comparison with any prior chest X-ray or CT chest available on the hospital PACS, with explicit note of new or resolving findings.
Findings
Systematic review of lungs, pleura, mediastinum, heart, hila, bones and soft tissues, and visualised upper abdomen. Negative findings stated explicitly where clinically relevant (e.g., "no pneumothorax").
Impression
One-line summary answering the referring clinician's question, with a short differential where appropriate (e.g., consolidation vs. atelectasis vs. mass).
Recommendations
Suggested next steps — repeat imaging interval, CT correlation, sputum / clinical correlation for suspected TB, or specialist referral.
Critical findings escalation
Time-sensitive findings such as pneumothorax, mediastinal widening, malpositioned ET tube, or massive effusion are phoned directly to the referring clinician within the 15-minute SLA.
