MedDream alternatives in 2026, matched to what you need

People leave MedDream for different reasons. The right alternative depends on which one is yours.

Last updated: October 2026 · By 5C Network

TL;DR

The best MedDream alternative depends on what you need. For AI and reporting in the same screen as the images, choose RadDream, the browser-based viewer inside 5C Network's platform. For a free, open-source web viewer to host yourself, choose OHIF (MIT license). For a free desktop viewer, choose Weasis (Windows, macOS, Linux) or Horos (macOS). RadiAnt is a paid Windows viewer with a free trial, not certified for diagnosis. For a subscription cloud PACS with a web viewer, choose PostDICOM (CE Mark Class IIb, per its site).

If you need a white-label, FDA-cleared, CE Class IIb viewer to embed in your own product, MedDream is often still the right choice.

When is MedDream still the right choice?

MedDream is still the right choice when you need a certified viewer you can embed. Made by MedDream, UAB in Lithuania, it is a web-based, vendor-neutral, white-label HTML5 zero-footprint viewer. Per its product page, it is FDA cleared for diagnostic use, including mammographic images, and CE Class IIb certified. It runs at 660+ B2B partners and 3,800+ server installations in 85+ countries. Stay with it if you:

  • ✓You build a PACS, VNA, EHR or portal and need a white-label viewer to embed and rebrand as your own
  • ✓You need a viewer that is FDA cleared for diagnostic use (including mammography) and CE Class IIb certified
  • ✓You read beyond radiology: cardiology, ultrasound, dentistry, ophthalmology or digital pathology
  • ✓You must run the viewer on your own servers or your own cloud account
  • ✓You want to integrate through URL, a Communication API or a Viewport API (React/Angular)

Why do people look for a MedDream alternative?

Usually because they need a different kind of product, not a weaker or stronger copy of the same one. The common reasons:

They want AI and reporting built in

MedDream is "AI & CAD ready": it integrates other vendors' algorithms. Reading rooms that want the AI, the draft and the report editor already in the viewer need a platform, not a component.

They want free, open-source code

Research groups and engineering-heavy teams often prefer a viewer they can host, read and modify themselves.

They only need to open studies on one computer

An enterprise web viewer is more than a single doctor, student or researcher needs. A desktop viewer does the job.

They need the archive too

A viewer needs a PACS behind it. Smaller centers often want storage, viewer and sharing from one subscription.

Which MedDream alternative fits your need?

If you want AI and reporting built in

RadDream by 5C Network

RadDream is 5C Network's browser-based DICOM viewer, built in-house, with nothing to install. Since October 2026 it is the viewer for every study on the 5C platform: 400+ specialist radiologists read in it every day across 20,000+ scans a day. Bionic AI findings appear on the image with a draft impression beside it, and the report editor is one click away. It covers MPR, curved MPR, 3D volume rendering, MIP and PET-CT fusion, plus hanging protocols, linked series, prior comparison and share links for referring doctors, across X-ray, CT, MRI, mammography, PET-CT and nuclear medicine.

Where MedDream is the better fit: RadDream comes with 5C's platform (5C reporting and 5C SuperPACS). It is not sold standalone, not white-label and not free to download, so it is not an option for a vendor that wants to embed a viewer in its own product.

If you want a free, open-source web viewer to self-host

OHIF Viewer

OHIF is the closest open-source match for MedDream's zero-footprint web viewer. Per OHIF's website, it runs entirely in the browser, is free and released under the MIT license, and is a program of Massachusetts General Hospital. Its GitHub lists DICOMweb archive support, MPR and MIP, segmentation, whole-slide microscopy and DICOM SR rendering.

The trade-off: OHIF's FAQ states it is not FDA cleared or CE marked. You supply the archive, hosting, security and support, which takes engineering time.

If you want a free desktop viewer

Weasis and Horos — and RadiAnt, which is paid

  • Weasis is free and open source (EPL 2.0 / Apache 2.0), per weasis.org. It runs as a desktop app on Windows, macOS and Linux or launches from a web portal, with 3D, MPR and DICOMweb support. Its site states the open-source distribution is not a certified medical device: no CE marking, not FDA cleared.
  • Horos is a free, open-source viewer for macOS, based on OsiriX and released under LGPL-3.0, per its official GitHub repository.
  • RadiAnt DICOM Viewer is a commercial Windows viewer, per its website, with MPR, 3D volume rendering and PET-CT fusion. It has a free trial with all features, then a paid subscription per PC. Its terms state it is not certified as a medical product and must not be used for diagnostic decisions or to report studies.

The trade-off: desktop viewers live on one computer and none of these three is certified for diagnosis, per their own sites. They suit students, researchers and doctors opening a study, not a reading room.

If you want a cloud PACS with the viewer included

PostDICOM

Per PostDICOM's website, it is a cloud PACS, CE Mark Class IIb diagnostic viewer and image-sharing platform. The zero-footprint HTML5 viewer runs in any browser, hosting is fully managed, and plans are monthly subscriptions by storage tier with a 7-day free trial.

The trade-off: it is a subscription service you use as is, not a white-label component you embed. If you also want AI and radiologists alongside the archive, see 5C SuperPACS and our guide to the best PACS software in India.

What about other enterprise, certified viewers?

Other commercial viewers compete with MedDream for OEM and enterprise deals, but we only list products whose facts we could check on the vendor's own site. If you need a white-label viewer with FDA clearance, ask each shortlisted vendor for its clearance documents, integration APIs and deployment options. Use MedDream's list above as your checklist.

How should you evaluate a MedDream alternative?

Start with what you are buying, then test on your own studies.

  • 1Component or platform? A viewer to embed in your product, or a reading room with AI and reporting? This one question halves the list.
  • 2Regulatory status for your use. Several free viewers state they are not certified medical devices. For primary diagnosis, ask for the documents that apply in your country; in India, that means CDSCO.
  • 3Where the archive lives. A viewer needs a PACS or DICOMweb archive behind it. Decide who runs it and where patient data is stored.
  • 4Total cost, not license cost. Free code still needs hosting, security and engineers. Subscriptions and platforms bundle those.
  • 5Test on your own studies. Open the same CT and MRI studies on your own network in each shortlisted viewer and time the reads.

Frequently asked questions

What is the best MedDream alternative?

It depends on why you are leaving MedDream, a certified white-label web DICOM viewer made by MedDream, UAB in Lithuania. If you want AI and reporting in the same screen as the images, RadDream, the browser-based viewer inside 5C Network's platform, puts Bionic AI findings on the image with the report editor one click away. If you want a free web viewer to host yourself, OHIF is the open-source standard (MIT license). For free desktop viewing, Weasis runs on Windows, macOS and Linux, and Horos runs on macOS. For a subscription cloud PACS with a web viewer, PostDICOM is CE Mark Class IIb per its website. If you need another white-label, FDA-cleared viewer to embed in your own product, MedDream itself is often still the right choice.

Is there a free alternative to MedDream?

Yes. OHIF Viewer is a free, open-source, browser-based viewer under the MIT license, and Weasis (Windows, macOS, Linux) and Horos (macOS) are free open-source desktop viewers. The trade-off: per their own documentation, OHIF is not FDA cleared or CE marked, and the open-source Weasis distribution is not a certified medical device, so clinical use is the responsibility of the institution. You also take on hosting, an image archive, security and support yourself.

What is the best open-source alternative to MedDream?

For a web viewer like MedDream, OHIF Viewer. It is a zero-footprint viewer that runs entirely in the browser, connects to image archives over DICOMweb, supports MPR, MIP, segmentation and microscopy, and is released under the MIT license as a program of Massachusetts General Hospital. Weasis is the strongest open-source option if you want a desktop viewer that can also be launched from a web portal. Neither is FDA cleared or CE marked, per their own sites.

Is there a MedDream alternative with AI built in?

RadDream, 5C Network's web DICOM viewer, has Bionic AI built in: findings appear on the image and a draft impression sits beside it, with the report editor one click away. On chest X-rays Bionic AI covers 74 pathologies with a 0.93 F1 score in peer-reviewed research. MedDream describes itself as "AI & CAD ready", meaning it integrates third-party algorithms rather than shipping its own. RadDream comes with 5C's platform (5C reporting and 5C SuperPACS) and is not sold as a standalone viewer.

Is MedDream free?

Per its product page, MedDream sells its viewer to software vendors, OEMs, system integrators, resellers and healthcare providers. For pricing and any demo or trial terms, ask MedDream directly. If you need a free web viewer, OHIF is open source under the MIT license.

MedDream vs OHIF: which should I choose?

Choose MedDream if you need a certified viewer: per its product page it is FDA cleared for diagnostic use, including mammographic images, and CE Class IIb certified, with white-label branding, multi-specialty support and on-premise or cloud deployment. Choose OHIF if you want free, open-source code (MIT license) that your own engineers can host, extend and customize; OHIF's documentation states it is not FDA cleared or CE marked, though its GitHub notes it has served as the basis for many FDA-cleared viewers built by others.

Who are MedDream's competitors?

MedDream competes with other web and enterprise DICOM viewers, and buyers often compare it with open-source viewers such as OHIF and Weasis, desktop viewers such as RadiAnt and Horos, cloud PACS products with built-in viewers such as PostDICOM, and platform viewers such as RadDream from 5C Network, which comes with built-in AI and reporting. The right comparison depends on whether you are embedding a viewer in your own product, running a reading room or just opening studies.

Is RadiAnt a free alternative to MedDream?

No. RadiAnt is a commercial Windows desktop viewer with a free trial that includes all features, then a paid subscription per PC; a perpetual license is offered for its CD/DVD version. Its terms state it is not certified as a medical product and must not be used for diagnostic decisions or to report imaging studies, so it suits viewing, research and teaching rather than primary reading.
